Multiple Forms Returns blank

Hello,
I’ve used Form Shortcode Builder from UMM and added two forms on my page. When submitting one, it resets the values of other forms to a blank.

FORM 1: is under “Edit Contact Info”
FORM 2: is under “Edit Website Info”
