MultiSite Issues 404 errors

By using out of the box default settings and looking all over the internet, I am having the exact same problem. The only option I can use is the Gallery > Network Settings. Any other options inside the network or inside a blog show the same exact URL: (substituting MyDomain instead of my real domain:
Result: 404 error every time.
