    Confirming the same issue here. Chased it a bit further and found that it is caused by not properly setting up the tables for multisite in network wide activation. It also stores the information about whether installation is complete in only one place, so per site activation will only work once as far as I can tell.

    The code appears to be pretty close to handling this correctly.

    I’m temporarily working around the issue by not using network wide activation and by manually creating the redirect tables.
