    Works great indeed!
    Now the question is, should be network activated or individually for each site? For now, I did only individual activation and works fine but I wonder whether network activation should be better.

    Another question: what happens when the plugin is de-activated? Do the files return to their original WP folders?

    You can do a site wide activation but you will still have to go into each site/blog and setup the folder settings. I will work on this to make it less pain full in the future.

    When the de-activate the plugin the files stay where they are and any new files uploaded will be put in the default WP upload folder with no subfolder. Does that make sense?

    Yes, I think also that both options above make sense.
    I tried to imagine different scenarios but this is the only way that makes really sense.

    Network activation and then site by site setup. I don’t think there is anything painful here.
    But that means that different sites will have different naming patterns for folders in the same year/month subfolder. Maybe this might be an issue and the naming patterns should be only set at network level? Just my 2 cents here.

    Regarding the plugin de-activation, I also think this is the way to go. It means that there is state before, during and after the plugin’s activation and that remaining tasks (ie grouping sub-folders together) should be done manually at any of those stages.
