Mutiple apache/php server behind load balancer

Hi, we used the plugin for many months without any major issues, but recently when we moved to a multiple server setup it keeps blocking admin to login and sends intrusion email for many logged in users. We cleared the table mentioned here: https://www.ads-software.com/support/topic/locked-out-of-own-site but that did not help. Any direction will be appreciated.
