When I visited the page on your site I couldn’t see the form there at all – even on regular PC.
I’ve imported shared form to my test setup and then noticed that the reason I couldn’t see it is because the form has the option “Enable logged-in submission only” enabled which means the form itself is only visible to logged-in users.
So on my test site I can see your form on every device I tested it with (laptop with two different browsers, iPad Air, Android-based smartphone) and it works fine but only if I’m logged in.
Did you test it on mobile as a logged-in user? Is the form supposed to be available to logged-in users only?
