My form disappears.

    Hi,

    I’ve just checked your page and seems to be working fine.

    The first step to solve issues with the form not appearing is to change the “Script Load Method” to “Direct” in the settings area located below the list of forms.

    Thank you the problem is fixed. The code has changed from form=”” to id=””
