Name, email and phone fields disappeared

    I’m implementing the plugin and 3 fields disappeared under the Apply Online heading: name, email, phone. I have tried switching the 2015 theme and I get the same result. It’s not on every job posting. The counselor/teacher position displays those fields. I added a new job, Drug & Alchol Therapist, and it doesn’t display on that page. Here’s a link

    Never mind. It appears that I needed to add the default fields of name, email and phone. I had done so in the one job posting and that’s why it appeared there. Issue is resolved.
