Name field Not populating with embed

Hello,
First of all, Thanks for the great plugin!
It’s a pain for us non techies getting these things to work!

I am having an issue populating the name fields.
As you can see the phone number and email address seem to populate perfectly.

I do not have pre-fill user info as we only have admins on this site.
I have tried using “name” instead of “firstName” and “lastName” but I get a 404 error. I also tried “full_name”.
