Name not displaying on feedback summary

First of all I’d like to thank you for the plugin. It worked from the moment I activated and I has a really cool style.
Here is my question: When I receive feedback, the name of the person is missing, although it is activated on the settings and appears on the form. In the “author” field I get the email address, but no name, so in case some writes and enter an email address like [email protected] I cannot know the name, hence I cannot answer by the name. Do you know how can I solve the issue?
I’m using WP 4.1 plus a Full Width SKT theme. I touched some of the code of the theme to hide some post info on the front-end, but in a very basic way, given that I know very little of php and css.
