Need Help Finding a "Survey Results into Post" Plugin

  • I am searching for a survey/questionaire plugin that I can embed into a page on my site. When my readers respond to the survery, their answers generate into a post.

    I know there is a plugin out there for me… I am just not typing in the right term(s) to find it.

    I want to be able to ask them yes or no questions, multiple choice, and fill in the blanks questions such as Name, URL, Favorite NYC Restaurant, etc…

    Any recommendations would be greatly appreciated!!!! ??
