Need help with an application form input page

    Not sure where to post this or exactly how to state the problem. I recently got handed the site managment and am learning about coding and everything about wordpress from scratch. Ive managed to pick and look up a lot on my own, but have run into one problem. On our page for application, where in a person inputs their infor and submits it to the site, I cannot edit the page. I looked through the plugin on the site and have spent a lot of time trying to figure it out on my own.

    Its in Japanese and all, but the basics should be the same. Where should I look for how to edit this page? Im lost and going crazy.
