need help with fields

You can create a form and add the fields you want to display, then copy the form’s shortcode into the pages.

what is shortcode then, i try so many and doesn’t work.

Then paste it into the page where you want to display the form. [wp-admin>pages>edit a page]

i know that, i need only one form shorcode to display page like in this picture only “skills” need to show, how can i do this?
