Need Help With the Form Email

Hi. My form has fields for the person’s first name, last name, email address, subject line and their message. I completely filled out all the fields I just listed and this is the email I received when I tested the form:

From: Riale <[email protected]>
Subject: [text text-399]

Message Body:
This is a form test.

How do I have the person’s first name AND last name appear in “From:” instead of just their last name only, and how do I have the actual subject line that they fill in on the form appear instead of “[text text-399]“?
