Need plugin: Send or download file after completing a form

I am looking for a plugin to perform the following functionallity:

– Adding a download file to a post
– Fill in a blank form with name and e-mail address
– Store of e-mail form information
– Release the file after completing the form via e-mail or download
– Track downloads

It is a function which is seen often on sites where whitepapers can be downloaded after suppling some personal information.

Is such plugin or phpcode available?
