Need simple registrtion plug-in with city, state, country.

Hello,

After searching the plug-ins for a few hours now I cant find a registration plug-in that allows users to enter city, state, country, contact info like phone, with email and occupation; like many many sites commonly do.

All I need is to know who is where, so anything simple that can ‘tally’ this data will work.
