Need to redirect to subsite after form submission

  • Hi,

    I am using a multisite network for one of my sites
    https://tenants.cliffordsinclair.com/test/

    There are number of subsites based on year and group. What I want is once the user click on submit button after selectng the house id and year, he will be redirected to relevant subsite.

    Can anybody guide me how can I do that?
