Need urgent help with doing registration page with CV attachments!

Hey guys, I am using the twenty 12 theme, not sure if it is the best theme for a university club website. what I need to do is to set up a registration page for an event, with google map, time, calender and most importantly, the questions and CV attachment (we have limited spaces so choose people based on CV). I managed to do everything besides CV attachment using the event registration 6.0 by david flemming, but really stuck! Due in 2 days!
