NEEDA PLUGIN: Best plugin for a banner/contact form?

  • Hi,

    I am trying to get people to order a free report which I then e-mail to them. On the home page of my website I want a banner underneath the navigation menu where I can have a form for name, email and website as well as a ‘SEND MY FREE REPORT’ button.

    A prime example of a WordPress-based website which already does this is Digital 22.

    If there is a plugin where this can easily be achieved then please let me know.
