Newsletter Plugin

I need an email opt-in form that does the following:

1) The list of emails can be exported and inserted into my email marketing provider database.
2) The email opt-in can be single or double opt-in
3) I can create different lists, I can have the list re-direct to a different url once the user has opted in.
