No delivery options available for United States (US) in cart

I believe the shipping options at checkout used to prompt the user for an address if one was not already set.

But we are now seeing “No delivery options available for United States (US)” at checkout with no prompt to fill out an address. If you expand the box anyway and fill in an address, you can get valid shipping options to show up.

We have “Default customer location” set to “Geolocation (with cache support)”. I’m wondering if we accidentally set a default country somewhere, and because the address isn’t entirely empty (United States is picked by default), it’s showing the no shipping options available message rather than prompting for a more specific address.

Perhaps the prompt to enter a shipping address should still appear if the submitted address is incomplete? Couple screenshots:
