No email received

The form submits, but it’s no longer sending/receiving the email response. I read that there are some bugs. I’m not sure if this is related? I’ve used the form for 5+ years. Currently, I’ve removed it.

I should mention that I’m using the Avada theme. I’ve deactivated plugins, etc. and still have the issue.
