No email received, and wrong send in

I am not receiving any emails on my [email protected] account whenever a user submits an email through our form at https://dev3.leadingpractice.com/contact/.
When I test the form and submit an email (as if I was a visitor), then I receive a notication email with a summary (which is good). Problem is, it says it comes from “The Global University Alliance” at [email protected]. See screenshot for mail received by form submitter: https://prntscr.com/jas1yx.
See screenshot for the mailer: https://prntscr.com/jas18h. All settings are correct for the mailer, but the resulting email notification to the user is completely wrong.
Please help?
