No "Enable WP Customer Reviews for this page" option available

    Check your screen options to make sure the WP Customer Review plugin is checked to turn it on (top right of your screen is the screen options tab)

    Hi. I have. It is. I have also since moved the WordPress site to root. Have deactivated and deleted the plugin and have reinstalled and activated the plugin with the exact same result.

    Do you have the same themes installed for both websites? Try switching the one not working to the standard Twenty Eleven theme to see if it displays then.

    You might also want to turn off any other plugins you have active to see if that is the culprit.
