No error, yet no image uploaded…?

I’ve been trying up upload images for a post, but every time i try (using the Flash and Browser uploaders) it finishes uploading, but there’s no image. Not even a thumbnail. It prompts me for image descriptions, titles, everything, like it thinks the upload worked. The image does not show up on the server. It doesn’t even make the folder path for the image (ex. uploads/2009/04/…).

Any ideas?
