No forms

I downloaded this plug in and no forms seem to appear on the wordpress site. I ahve looked for the forms everywhere. I am trying to create a form with paypal payment subscription. Have I downloaded the wrong forms? Is there another plug in that needs to be downloaded with this plug in? Is this a paid service? if so how can i pay
