In your case it seems to be a cache problem, because the toggle Fake Internal Featured Image is still disabled according the data you sent me. Try to clean your cache and run the steps below.
To other users, it’s probably caused because an old plugin feature was deprecated, as announced before, and replaced by a new one. This is what you guys should try:
1) access FIFU settings, metadata and…
2) enable Clean Metadata (it will be automatically disabled after some seconds)
3) enable Fake Internal Featured Image toggle.

Disable “FIFU settings > Performance > Lazy Load”.
There is a conflict with Jetpack Lazy Load.
You can’t have both enabled at the same time.
It should solve your issue.

I spent days to disable plug-ins / caches / themes and eventually I found out the cause. Latest version of featured image from URL is not compatible with my theme. I have rolled back to an older version and it works right away.
Here is my site: https://www.51sec.org – using version 2.7.0 , working very well. I tested it with 2.7.4, some old pages works fine, but not latest pages.
my backup site: opc1.51sec.org – using version 2.7.8 , not able to show the external image.
My second backup site : gcp1.51sec.org – using latest version too 2.7.8, not working.So , at this moment, I have rolled back to 2.7.0 and it resolved my issue. I have to find out a way not to auto-update my plug-ins now.

I just installed Startup Blog theme here and it’s working perfectly with FIFU. So it should be something else.
By the way, did you disable Lazy Load in FIFU settings?
Your fix works, but I have a few hundred websites to update. Is there any way to automate from command line?
Post > Edit Post > Update also works. So I tried wp clis “wp post update” command. Unfortunately, that does not work. Any suggestions?

Unfortunately there aren’t command line features. It was never required before. But I think it would be amazing to integrate FIFU with WP-CLI.
In a few days I should start a campaign to raise funds to include new functionalities in the free version. If that works, be prepared to see a plugin MUCH better than it is today.
