I fixed the compatibility. And currently sorting out a problem with a domain, the previous owner totally abandoned the plugin and it takes time to sort out some things.

You are right @nikolaystrikhar thank you for the excellent work you are doing so far. The form is much more improved and now compatible with this current edition of WordPress. I tried it out and also left a review which is still in moderation.
