To confirm, have you also ignored the CSS and JavaScript asset resources provided in our knowledgebase article for W3 Total Cache? If not, please try to set it up, and see if that resolves the issue.

Can you exclude the checkout page from being cached and let me know if it helps?
Usually issues like this are caused by a misconfiguration in a caching plugin.
