Line # 88 has the following CSS rule:

media=”all”
input {
-webkit-appearance: none;
}

You need to comment out this rule or add a custom CSS within the WHMCS plugin style block for:

#order-boxes input[type=”radio”] {
-webkit-appearance: radio;
}

If that doesn’t work add an !important before that closing “;” of the CSS rule.
