Non-existent user acct deletes other non-existent user acct?

Using v2.11, today an activity was logged stating:
WordPress deleted test on [my blog name]
There isn’t a “WordPress” account, nor was there a “test” account. I’m willing to provide SQL dump or whatever else you’d need to track down this apparent bug.
