not all photos show when i use quick upload!

first my button UPLOAD IMAGE doesnt work at all, second when i used all attachment button pictures dont have effect (ligtbox), when i use quick upload for 6 images it only put 2 or 3 others are blank.
for now my option is only to use quick upload for several times until i got that picture that i wont into gallery.
