not all posts generate notification

After some complaints from a subscriber, I compared the list of published posts with the list of Sent Mails, and found that of the last 10 posts, only 6 generated notifications. I’ve confirmed in the plugin settings that the categories of the posts are checked off in the send notifications list, and previous posts in those categories have generated notifications.
Any ideas on what could be going on here? I have only about 20 subscribers, and the problems seem to occur only since July. Emails are set to be sent immediately upon publish, not via a cron job. And just to be sure I’m clear–this is not a problem with the email not being received; rather, the plugin reports show no evidence of a notice being generated from the publication of these posts.
Some possibly related questions:
1) If a post is accidentally published in a category (such as “uncategorized”) not set to generate a notification, but then later edited to be placed in the correct category, would it being in the new category trigger a notification email?2) How does the plugin handle posts that are set to publish in the future? I have one post that shows a published date of 6/16, but the notification about it is dated 6/3.
3) When is a Sent Mail report generated–after the post is published, or after emails are successfully sent? That is, if a post was published but something went wrong with the send process, would any report be generated?
