Not Getting Notification

I really like the ease with which the contact form can be set up
However, I am not able to get email notification after following the instructions posted in the forum.
I placed my personal email address in the Send To box. I tried it with and without [smfield print=”email”] in the reply to box. I put my name in and left it out. I also put the subject in and left it out. Under none of the conditions did I get a notification.
Also it says “Set it to unique so you can manage and find multiple notifications easily.” It is not clear what that means or how to do that.
I hope you can provide a more detailed explanation as to what I might be during incorrectly.
