Not loading on the frontend with oxygen builder

I tried it with different forms, as well as via oxygen component and shortcode. nothing has worked so far. I am using oxygen 4.6

Thank you Mark! I have submitted a ticket via the support form. In oxygen’s backend, it give an error saying:

Uncaught TypeError: i.checkValidity is not a function on line #1 in https://staging-poly.metricdesign.net/why-polymaker/?ct_builder=true&ct_inner=true&oxygen_iframe=true

Not sure if they are related.

In case anyone else has this issue, there was a duplicate element ID in the DOM:

ID’s should always be unique.
