In order to investigate things further, could you please share a copy of your site’s System Status? You can find it via WooCommerce > Status. Select Get system report and then click the Copy for support button.?You can either paste it in your response here, or use https://pastebin.com/ for pasting it there and sharing it with us (as it provides additional paste settings).

Also, could you please navigate to WooCommerce > Status > Logs and from the dropdown on the right side, please see if there are any logs starting with the words fatal-error.

If so, please select View and copy and paste them into your reply here.

Can you provide your system status report in text format, please? That will help to investigate your case.

You can also navigate to WooCommerce > settings > general >Currency options > number of decimals. Do you have that setting set to 4 decimals?

Also, which payment gateway are you using to charge the order, and to process the refund?

Thanks for confirming that, and for the System Status Report!

I believe this is a known result for stores using 4 decimal places, as the refund module appears to default to using 2, as noted here: https://github.com/woocommerce/woocommerce/issues/30263

It appears this has already been identified as a necessary enhancement and an update is in progress (testing phase) here: https://github.com/woocommerce/woocommerce/pull/34641

In the meantime, it appears that using 2-decimal place settings will be the only workaround.
