    thanks for reaching out and for the screenshot!
    In order to help us understand the issue a bit more I am going to need some additional info:
    1. WP version
    2. plugin version
    3. link to your blog

    A few months ago this was fixed and users experiencing this had an old version of WP or our plugin installed. Also, keep in mind that other plugins such as WP Total Cache or similar can cause problems such as the one you are having.

    Yes i am using wp total cache but why this not showing proper images

    thanks for getting back to me!

    I have checked out your blog and could it be you have manually edited some of the related posts via the orange “Edit Related Posts” button?
    The thing is, when you manually edit the post, this should get stored in a cache in a way, so when using cache related plugins it can cause some issues. But to be honest, it could be something else.
    Let me know what happens when you click “Edit Related Posts” and you remove or add a post to your selection (don’t forget to Save and Close) and then re-add the same post.
