Not Receiving Some Forms

For some odd reason, some forms go through successfully, but some don’t.

I know of 2 people who sent in the exact same form, but I never received them via e-mail, and they never showed up in the Database XML Back-up.

This is very important for my business. I could have lost lots of potential client forms and I wouldn’t even know about it.

What’s causing this?

The site is located at https://www.flowersitters.com and there are lots of FormBuilder forms on several pages of the website.
