The modal window has the form in the content, but the form has property display: none. I don’t know why.

You can try to add on the page CSS code as

#wow-modal-overlay-1 #gform_wrapper_1 {
    display: block !important;
}

• In the site dashboard:
• Go to the page ‘Appearance’->’Customize’
• Click ‘Additional CSS’
• Add CSS code
• Click ‘Published’

On the page, you have two forms with the id gform_wrapper_1. Try to create a duplicate of the form gform_wrapper_1 with another ID and insert into the modal content
