Not working

Hi, the button is not working. It shown on the page I want it by putting [thankyou] but it doesnt do anything when click… could you please tell me exactly what to do to fix this? The page is this:

I read something about the database and the create issue but I know nothing about that so dont know even how to check that or fix it if is wrong…
