Not working – Clean and simple contact form

Hi I downloaded and activated the plugin called Clean and simple contact form. Then it says to paste this into a page [cscf-contact-form]

I did that saved the page and published it. When I click the contact button on the top menu of my blog it goes to a blank page. What am I doing wrong? I thought all I had to do was activate a plugin and paste the short code into a page. Help I need a contact form on my blog. I have actually tried several different plugins all with the same results
