    Thanks for providing those details about your issue.

    Sounds like your Gravity Form still has a “Quiz” field but you’ve since uninstalled the Gravity Forms Quiz add-on. The quick fix would be to replace the “Quiz” field with a Radio field (or reactivate the Quiz add-on).

    I’ll open a ticket to look at fixing up the PHP notice as well.

    Hi Jake,

    Is there a way to reach you outside of this forum? I would like to send you my form so you can look at it. I did re-activate the Quiz plugin, but the issue still remains (even after caches cleared).

    I don’t even see a quiz field in the form.

    Please advise at the soonest convenience.
