Notification – cannot add all post categories

Hi,
I’m trying to create an notification for a subscription group for all categories in a site. I select each category on the notification creation page (along with filling out the rest of the form), and hit submit. Unfortunately, 4-5 of the categories are not included. The same behavior occurs when I attempt to edit the notification to add these missing categories.
Is there a limit to the number of categories you can add to a notification? Why isn’t there an “all categories” checkbox? I believe this is a bug.
Also, why can’t I create a custom title for the notification email?
