Older posts not working

Have you made several posts ‘sticky’ and are only showing a smaller number of posts?

try:

<?php $paged = get_query_var( 'paged' ) ? get_query_var( 'paged' ) : 1;

(regardless what the codex says, this sometimes seems to work better)
