One form – choose from multiple lists – Broken how to fix

Seems like 2-3 years ago we setup a Gravity Form for a client and there were 5 Constant Contact lists a subscriber could choose from.

I believe we added some code and in GF created a checkboxes field and under advanced set “Allow field to be populated dynamically” to be ‘Emaillists’

Worked great for some time but now it’s now broken and not working and I’ve been racking my brain and searching on how to do this or maybe there’s a new way?

I have the plugin installed and we can create feeds to a form but how can all of these feeds show on the form as a choice?

Is it still possible to have one GF form and choose more than one list to subscribe to via Constant Contact?
