One of my forms Not loading on Front end

  • I have two forms, one complicated form which displays just fine, and a basic contact form which will only display in the back end when inserting into a Page, or if I edit it. It will not display fromt the Front end. Any suggestions
    I have WordPress4.7.4 , Custom Contact Forms ver 7.8.5, Woocommerce 3.0.5
