Optional product in product variant

Hello,

we created a variant product with two variants. Now we want to add another optional product.
Like on this page: https://eu.balibodyco.com/de/products/instant-tan
There you can choose between two colors and then optionally add an extra product.
Is there a way to do that smoothly?
