Order Forms – Multiple Products

Are there any extensions that allow the creation of an order form out there? Setting up a fundraising site that will have multiple causes and I’d like a page that lists all the causes, allows the filling in of a $$ amount next to each cause then a way to add each of those to the cart. Very similar to how this site performs the same task give guide donation form
This would be in addition to the more traditional method of adding items (causes) to the cart.
