"other" field duplicates

I am using Simple Donate to create three donate options on one page: one for monthly donations, another for single donations and a third option all using radio buttons and an “other” amount option.
Because I use the [simple donate] shortcode generator more than once on the same page, I’m getting a duplicate field for “other” on each shortcode use. I’m using it three times and now getting three “other” fields on each donation option. Is there a fix for this? Everything else seems to work fine.
