1. i have used free version of forminator but not able to add shortcode in elementor. i m using elementor builder
I just checked the mentioned page and could notice the the form is loading fine. What exact issue you have with adding the shortcode? Please provide further information.

You could use a Select field with calculation enabled in the Select field and visibility conditions to make it work.
For example, create a select field for Q1, where “Interface Design” would be one of the options in that select field and could use visibility conditions to show up options for Q5 as a checkbox field that could be selected.

3. Column layout and other option for typing is possible or not in your plugin
You can drag and drop the fields in the form to align it as a columns. Other than that it’ll require custom CSS to design it according to your requirements.
